Hair topper or wig? Choosing the right option for alopecia and bald patches

If you have bald patches or alopecia, choosing between a hair topper and a wig can feel overwhelming. The short answer: a wig covers your whole head and suits total or near-total hair loss, while a hair topper covers one area and blends with the hair you still have. For patchy hair loss, a thin crown or a sparse parting, a bespoke topper is often the more natural, comfortable choice.
What is the difference between a hair topper and a wig?
A wig is a full head of hair worn over your own. It replaces everything, so your own hair, if you have any, is tucked away underneath.
A hair topper is a partial piece that covers only the area that needs it, such as the crown, the parting or a patch. Your own hair stays part of the look and is blended into the topper, so the two read as one.
Within toppers there is a big difference between ready-made pieces and a bespoke system:
- Ready-made toppers come in standard sizes and usually attach with clips. They are a quick option, but they are not shaped to your head and clips can pull on fine or fragile hair.
- Bespoke skin toppers are made for you from a cast of your scalp, and fitted and blended by a trained stylist.
Hair topper vs wig: which suits you?
| Wig | Ready-made topper | Bespoke Skin Hair Topper | |
|---|---|---|---|
| Best for | Total or near-total hair loss | Mild thinning in one area | Bald patches, alopecia areata, thin crown or parting |
| Coverage | Whole head | One area | Exactly the area you need |
| How it is held | Worn over your own hair, often with a cap | Clips | Adhesive, with no shaving, beads or net |
| Fit | Standard sizes | Standard sizes | Cast to the shape of your scalp |
| Feel | Can feel warm | Can pull on fine hair | Lightweight and breathable |
| Who fits it | Often yourself | Yourself | A trained specialist |
We will always be honest with you. If you have lost all or almost all of your hair, a wig may be the right answer, and we will tell you so at your consultation.
Why a bespoke skin topper can feel so different
Our Bespoke Skin Hair Toppers are made using the ICONIC SKINN™ system, which Jayne Bell trained in with Iconic Hair in Peterborough. Rather than sitting on top of your hair like a hat, the topper is designed around your scalp and the areas where you want coverage.
- Cast to fit you. A cast of your scalp means the base sits flat and follows the shape of your head.
- A natural-looking parting. Human hair is injected into a thin polymer base, so it appears to grow from your scalp wherever you part it.
- Breathable. The base has small holes built into its design, letting air flow through while you wear it.
- No shave, no beads, no net. It is applied with adhesive, so your own hair stays part of the picture.

With the right care and regular maintenance, a bespoke topper can last up to 12 months. At each maintenance visit Jayne removes the system, cleanses your scalp, refreshes the adhesive and refits it.

What if my hair is thinning rather than patchy?
If your hair has thinned all over but you still have a good amount of it, neither a wig nor a topper may be the best fit. Mesh Hair Integration blends matched human hair through your own via a fine, breathable mesh, and it often suits all-over thinning better. Our guide to thinning hair in women and your non-surgical options explains the difference.
Frequently asked questions
Can you wear a hair topper with alopecia?
Yes. A bespoke skin topper is often an excellent option for alopecia areata and bald patches, because it does not rely on your own hair to hold it in place. It is applied with adhesive and blended into the hair around it. Jayne will check it suits your hair loss at your consultation.
Is a hair topper better than a wig?
It depends on how much hair you have lost. A topper is usually better for patchy loss or thinning in one area, because it works with your own hair and feels lighter. A wig is often better for total or near-total hair loss.
Do hair toppers damage your hair?
Clip-in toppers can put tension on fine or fragile hair over time. A bespoke skin topper is applied with adhesive rather than clips or beads, and regular maintenance visits keep your scalp clean and healthy.
How long does a bespoke hair topper last?
With the right care and regular maintenance appointments, a Bespoke Skin Hair Topper can last up to 12 months.
